Reconstrucción de hombro en paciente quemado con colgajo de Latissimus Dorsi. Caso Clínico

Introducción: Las quemaduras eléctricas forman parte de un gran reto quirúrgico y reconstructivo para la cirugía plástica, con altos índices de morbi-mortalidad. En el Ecuador los casos son prevalentes y afectan en su mayoría al sexo masculino. Caso clínico: Se presenta el caso de paciente joven de 21 años que sufre quemadura eléctrica con alto voltaje del 10% de superficie corporal, punto de entrada mano derecha con secuela de axonotmesis grado III del nervio cubital y radial, y punto de salida hombro izquierdo con quemadura de tercer grado, área más afecta. Se realizaron 15 intervenciones quirúrgicas para debridamiento de tejido necrótico y utilización de injertos de espesor parcial para áreas expuestas. Paciente presentó durante hospitalización cultivos positivos para pseudomona por lo que se manejo con antibioticoterapia de amplio expectro, taquicardia sinusal y depresión. Conclusión: Al obtener tejido granulación y tejido vital se decide utilizar el colgajo de Latissimus dorsi para reconstrucción de hombro con excelentes resultados de funcionalidad, mostrando la utilidad de este colgajo en pacientes con quemadura eléctrica.
