Ovarian serous cystadenoma is a type of tumor derived from the superficial coelomic epithelium. Giant ovarian cysts are those that measure more than 10 cm in diameter. Serous tumors are commonly 70-80% benign, 5-10% have some malignant potential, and 20-25% are malignant. Only 10-20% of cases are bilateral. They occur in adults of all ages, with reported mean ages of 40 to 60 years1. We report the case of a 43-year-old perimenopausal woman with a giant left ovarian cyst measuring 15.5 x 12 x 3.5 cm and weighing 527.8 grams with a smooth capsule with an important vascular network containing serous fluid and a 72.6-gram right ovary measuring 5 x 3.5 x 2.5 cm, with a smooth, whitish external surface with paraovarian cystic areas between 4 and 0.8 cm in greatest diameter. She says that she comes to the consultation for presenting abdominal pain radiating to the pelvic region for 3 days prior to her hospital admission. Taking into account the intensity of the pain and the possibility of ovarian torsion, an operative laparotomy was indicated in which left partial oophorectomy was performed plus cystectomy plus drainage of left ovarian cysts plus lysis of adhesions from the left tube to the ovarian cyst, an oophorectomy of the right ovary. In the histopathological examination, the cyst was confirmed as serous cystadenoma of the left ovary + right polycystic ovary. She had a favorable post-operative evolution.
